The review of Hamyln et al collects information suggesting that the adrenal glands secrete ouabain (or a closely related isomer), which circulates in plasma at nanomolar levels in healthy subjects and at higher concentrations in patients with disorders of blood pressure or fluid and electrolyte balance [1]. The authors note experiments in rats showing that chronic ouabain administration can induce hypertension. Based on this evidence, Hamlyn et al. propose that ouabain (or an isomer of it) might contribute to the long-term regulation of arterial pressure [1]. They suggest also that defects in the renal excretion of ouabain, whether inherited or acquired, may account for raised circulating ouabain levels in certain patients with essential hypertension and induce, or contribute to, the development of their hypertension [1].
