This chapter reviews the current literature on the presence of the human immunodeficiency virus (HIV) in semen. The 3 major issues include: 1) the localization of HIV in semen; 2) the incidence of infectious HIV in seropositive homosexual men; and 3) genital tract inflammation as a potential co-factor in HIV transmission. White blood cells are the primary producers of HIV in male reproductive tract secretions and may themselves be the vectors of HIV transmission. Semen increases the infection rate in those in the more advanced stages of HIV. Additional co-factors that may influence the sexual transmission of HIV through semen include physical barriers the presence of immunosuppressive factors in the male reproductive system antisperm autoimmunity genital tract infections with other viruses anti-HIV antibodies in sexual secretions and genetic factors.
