The first patient was a male Negro aged 22 years who 20 days before admission complained of "intense headache, slight fever, vomiting, and drowsiness". He also suffered from a sore throat and vertigo. On examination the patient was conscious, but had a staggering gait and a number of ocular abnormalities were present. An electro-encephalogram revealed a possible parietotemporal lesion, and it was accordingly decided to operate. By exploratory aspiration pus was obtained, and an abscess was located " 2 cm. below the cortical surface of the left parietal lobe. . .". Post-operatively Lugol's iodine was administered to the patient in the dosage of 10 drops 3 times daily for 1 month. At the end of this period of treatment he had recovered so well that he was considered fit to be discharged.Histological examination of biopsy material revealed tissue of an inflammatory nature containing multi-nucleated giant cells and also numerous olive-green hyphae. Culture showed these to belong toCladosporium trichoides. The second patient was also a male Negro. He was aged 29 years. 9 months before admission he had experienced a motor-car accident as a result of which his face was lacerated. Subsequently, symptoms of meningitis developed. On admission he was ataxic, his speech was impaired, and he exhibited nuchal rigidity and ocular abnormalities. He died 2 days later.Histological examination of necropsy material from the brain revealed " an intense granulomatous meningitis, with numerous, multinucleated giant cells ". Fungal hyphae were abundantly represented and the fungus was morphologically identical withC. trichoides. The description of these cases is fallowed by a review of the literature on this subject, which is now considerable, and also an account of experimental inoculations in animals with the fungal strains obtained from one of these patients and with strains of other related species, includingFonsecaea.
