Neoplasms sometimes produce symptoms distant from the tumor itself without any direct tissue invasion or tumor mass. Symptoms and signs, caused by protein hormones, their precursors, or cytokines produced by the tumor are defined as paraneoplastic syndrome. 1 The spectrum of clinical features in patients with paraneoplastic syndromes associated with primary lung cancer ranges from mild systemic cutaneous disease to severe neuromyopathic disorders. 2 Because its diagnosis is often one of exclusion, its recognition may be difficult or late.We present a case of non–small-cell lung cancer with generalized hyperpigmentation, preceding the diagnosis of the patient’s malignancy by 2 years. Hyperpigmentation resolved after combination chemotherapy, accompanied by regression of primary tumor and relieving of itching.
