To evaluate whether the nicotine and CO content of cigarette smoke was related to the risk of nonfatal 1st myocardial infarction in young men, 502 cases were compared with 835 hospital controls, aged 30-54 yr. The estimated risk of myocardial infarction increased with the number of cigarettes smoked; the relative-risk estimate for current smokers was 2.8 (95% confidence interval, 2.0-4.0). The risk did not vary according to the amount of nicotine or CO in the cigarette, and the mean amounts of both substances per cigarette were similar for the cases and controls. Men who smoked the newer cigarettes with reduced amounts of nicotine and CO did not have a lower risk of myocardial infarction than those who smoked cigarettes containing larger amounts of these substances.
