A recent report (2) has confirmed the association between ele-vated plasma levels of intestinally derived triglyceride-rich lipoproteins and coronary artery disease. Acarbose, a potent intestinal glucosidase inhibitor, reduces the availability of glucose for intestinal absorption and, therefore, suppresses postprandial plasma glucose and insulin levels in diabetic subjects (5). It has also been reported that postprandial triglyceride levels can be reduced by acarbose treatment (1). Thus, we examined the effect of acarbose on plasma lipid levels, especially on remnant-like particles (RLP, lipoproteins without apolipo-protein A3 and B100) in non-insulin dependent diabetic subjects (NIDDM).Thirty-one NIDDM patients (15 male, 16 female: 24 on diet, 7 on sulfonylureas) aged 51(SD) years were studied. After more than 3 months of stabilized blood glucose control, acarbose (150 mg/day in 3 divided doses) was administered for one month. Separation of RLP fraction from whole plasma was achieved by using an immunoaffinity column containing antihuman apolipoproteins (apo) A1 and B100 monoclonal antibodies (3). The characterization of unbound fractions has shown that they represent chylomicron and VLDL remnant-like par-ticles.
