The most frequent and serious complication of continuous ambulatory peritoneal dialysis (CAPD) is peritonitis (1). Many different bacteria have been isolated from infected peritoneal effluent. The source of the responsible organism is either the external or the internal colonizing flora. Examples of the former group are, in order of frequency, Staphylococcus epidermidis (30%) and Staphylococcus aureus in about 10% of cases. In the latter group, Escherichia coli is a responsible pathogen in about 15% of CAPD peritonitis (2). However, infection with Bacillus cereus has only scarcely been reported and is not included in a recent overview of possible pathogens (2). We describe 2 patients on CAPD who developed B. cereus peritonitis. The bacteria relapsed even after adequate antibiotic therapy and, in both cases, B. cereus infection led to replacement of the CAPD catheter.
