This is one in a series of reports by the Antimicrobial Agents Committee of the Infectious Diseases Society of America (IDSA) that address issues in antiinfective therapy [1-4]. Glucocorticosteroids (which for convenience will be called steroids in the rest of this statement) have many therapeutic uses, including treatment of selected infectious diseases or their complications. Guidelines will be given for the systemic use of these agents in the treatment of certain infections. Topical therapy with steroids is not addressed, nor is use of steroids to treat certain complications of the infection process in general (e.g., nephrotic syndrome [5], elevated intracranial pressure or other neurologic sequelae [6], hemolysis [7]). Steroids exert major immunosuppressive and antiinflammatory effects in many organ systems [8], impairing antibody production and cell-mediated immunity and affecting many cells of the immune system. T cells are sequestered transiently; their movement is markedly disrupted by steroids [9, 10]. Monocyte traffic, including egress from the circulation, is impaired. The production of most cytokines by lymphocytes and mononuclear phagocytes is inhibited [I 1]; further, the effects of cytokines on some target cells is also blocked [12]. Steroid therapy transiently reduces polymorphonuclear inflammatory responses and decreases lymphocyte, monocyte, eosinophil, and basophil counts in peripheral blood. These effects on cells are most evident if steroid doses
