The Patient Protection and Affordable Care Act has drastically improved insurance coverage in the USA and created a framework for payment reform. As Philip Gorelick points out, with these drastic changes come great opportunities and daunting challenges. Although the USA is actively addressing these challenges, we should recognise that many other countries already have lowercost and higher-performing health-care systems with various health system structures, including fee-for-service structures. Specifically, many other countries provide comprehensive health-care coverage, keep costs of tests, procedures, and medications lower, and maintain better measures of health and health system performance than the USA, all with a neurologist workforce that is smaller than that of the USA. Perhaps more international comparisons of neurological care and outcomes are needed so that the USA can learn the valuable lessons already learned by the international community.
