Splanchnic perfusion following open-heart surgery

为了解体外循环后内脏灌注量的变化，测定了心内直视手术后患者胃粘膜pH值(Phi)和肝静脉血氧饱和度(HV&GT；O2)。在入住重症监护病房(ICU)后6、12和24小时对血样进行分析。结果如下：1)胃液pH值由6小时的7.21显著升高至12小时的7.31，24小时恢复至7.37；2)S、HV&GT；O_2由6小时的48%显著升高至12小时和24小时的57%；3)心脏指数>4 L/分钟/米^2，混合静脉血氧饱和度>70%。尽管心输出量充足，但体外循环后内脏灌注量减少，并在ICU入院后24小时内恢复。结论：胃液PHI和S、HV&GT；O_2是心内直视手术患者术后内脏灌注量的有用参数。

To determine splanchnic perfusion after cardiopulmonary bypass, gastric intramucosal pH (pHi) and hepatic venous oxygen saturation (S_<HV>O_2) were measured in patients following open-heart surgery. Blood samples were analyzed at 6,12 and 24 hours after the admission to an intensive care unit (ICU). The results were as follows : 1)Gastric pHi increased significantly (p<0.01) from 7.21 at 6 hours to 7.31 at 12 hours and recovered to 7.37 at 24 hours, 2)S_<HV>O_2 increased significantly (p<0.05) from 48% at 6 hours to 57% at 12 hours and 24 hours, and 3)cardiac index was more than 4 L/min/m^2 and mixed venous oxygen saturation was over 70%. Despite sufficient cardiac output, splanchnic perfusion decreased after cardiopulmonary bypass and it recovered within 24 hours afrer admission to an ICU.In conclusion, gastric pHi and S_<HV>O_2 are useful parameters for postoperative splanchnic perfusion in patients with open-heart surgery.
