Clinicians receive inconsistent instruction on how to use patients’ racial identities in clinical documentation and decision making1, 2 and often document this information without clear reasons or an understanding of its relevance. Routine documentation of racial categories is rooted in the mythology of inherent biologic differences between racial groups, especially between Black and non-Hispanic White people. 3, 4 Such myths, founded on medical racism and White supremacist ideology, have been used to justify historical atrocities, including enslavement of people of African descent, 5 and continue to normalize disparate treatment of Black and other racially minoritized patients by the US health care system. 4
