ANNUALLY, MORE THAN 100,000 TENDONS are com-promised as a result of traumatic injury, chronic disorder, or removal for use in ligament reconstruction. Treatment of these disorders has had varying success depending on the severity of the injury. 1–4 Intervention for tendinopathies has been shown to produce good or excellent outcome in a majority of cases. 1, 4, 5 For repair of quadriceps tendinopathy, the strength in the repaired limb is equivalent to that of the contralateral limb. 6 However, the appearance of the repaired tendon, using ultrasound or magnetic resonance imaging (MRI), is abnormal. 7 When tendinous ruptures are repaired, the tendon is not typically restored to preinjury strength or size. 2, 3, 8 The repaired tendons on follow-up examination are significantly larger than the contralateral tendon, 8, 9 and the strength of the in-series muscle is compromised by as much as 16.6%. 2 In addition to these injuries, large patellar tendon defects are created when the tendon is used as the graft for cruciate ligament reconstruction. The gap created in the patellar tendon is not repaired. Short-term follow-up found that the area between the remaining patellar tendon struts decreased, but a 2-mm gap was present for as long as 27 months. 10 Incomplete tendon healing can lead to chronic pain, activity limitations, and tissue degeneration.Several studies have focused on improving the success of tendon repair. Many of these studies used a functional tissue-engineering approach, which determines mechanical and structural requirements for a tissue and uses them as a model for the repair design. 11–13 This approach has resulted in a significant improvement in tissue quality compared with natural repair, but the repair tissue is still inferior to normal. 14–16 Typically the components of the repairs are a combination of cells with a scaffold or delivery vehicle.
