There have been many suggestions that virus infection may be involved with the onset and/or the progression of diseases of currently unknown etiology, including Type I diabetes (IDDM)[1]. Several independent studies using either human patients or animals have documented virus-induced pancreatic islet disorders or have identified a virus within disordered islets [2-8]. However, no unequivocal association has yet been established between IDDM and infection by a single virus. The possibility is now emerging that infection of~ cells by anyone of a group of apparently unrelated viruses (for example: coxsackie, rubella, mumps, cytomegalovirus) may either initiate a pathogenic process leading to IDDM and/or represent a final insult to precipitate clinical diabetes in genetically susceptible individuals. Because virus isolation from islets of diabetic patients has been the rare exception rather than the rule, we believe it is important to evaluate a link with chronic or persistent virus infections. Our objective, in this review, is to consider the molecular and cellular basis for the interaction between virus and host during a long-term chronic or persistent infection. We have included experimental results from the mouse model of persistent infection with lymphocytic choriomeningitis virus (LCMV) to illustrate some recent progress and to outline problems that remain to be resolved for a complete understanding of the pathogenic processes involved with virus-induced diseases.
