In 1987, Ducimetiere and his team coined the term “the French Paradox” to refer to the fact that the French have among the lowest rates of cardiovascular disease in the world despite having among the highest intakes of saturated fat (1, 2). Since then the MONICA project, aimed at standardizing and monitoring trends in cardiovascular diseases, showed that rates of coronary deaths in France were similar to rates in adjoining countries of Southern Europe (3). However, what lingers in the American consciousness is the belief that the French eat better, drink better, and live healthier lifestyles than Americans. Perhaps a more realistic assessment is that both countries can learn valuable lessons from one another about the sociocultural and economic factors that influence lifestyle.The paper by Malon and colleagues (4) in this issue of the Journal reports on the results of an extremely rigorous national survey assessing adherence to French nutritional recommendations. Major findings were that of 1 physical activity and 12 nutritional recommendations, the average French adult adhered to about 50% of them. Those most likely to be non-adherers were the young and middle-aged and the economically disadvantaged. These data suggested to the researchers that perhaps education alone is not enough to change eating behavior but that targets of intervention should also focus on the availability and costs of healthier food alternatives.
