As cochlear implant (CI) candidacy guidelines expand to consider children with more residual hearing, the use of a CI and a hearing aid (HA) at the non-implanted ear (bimodal devices) is increasing. Given that CIs and HAs may differ in their ability to transmit speech cues related to segmental perception (phonemes) and suprasegmental perception (fundamental frequency and stress), an examination of the unique effects of electrical and acoustic hearing as related to speech perception is warranted. This case study aims to examine the contributions of acoustic and electric input to segmental and suprasegmental performance for a pediatric bimodal device user who is a borderline bilateral cochlear implant candidate. Further, the subject’s spoken language performance is compared to those of a large group of children with prelingual severe-profound hearing loss who used two devices from a young age, at least one of which was a CI. The subject completed a battery of speech perception tests in CI-only, HA-only and bimodal conditions. Performance in all three device conditions were generally similar across tests, showing no clear device-condition benefit. This bimodal user’s speech perception and language scores were average or above-average when compared to a group of pediatric CI recipients. A careful examination of both segmental and suprasegmental speech perception, and of spoken language skills should be conducted to determine the broad-scale performance level of bimodal recipients. This may be especially important when deciding whether to move from bimodal devices to bilateral CIs.
