Addressing the Challenges of Clinical Research for Freezing of Gait in Parkinson's Disease.

A recent Scientific Panel Discussion led by the Scientific Issues Committee of the International Parkinson and Movement Disorder Society has concluded that a “systems biology” strategy should be implemented to advance our understanding of the pathophysiology underpinning gait and balance disorders in Parkinson’s disease (PD). Critically, the failure to appreciate the neurobiological processes associated with these challenging symptoms is seen as the most significant barrier to the development of more successful therapeutic approaches. The Scientific Panel has also proposed that there should be a focus on the development of testable experimental models probing the interacting circuits and networks that control gait and postural balance (see details in Bohnen et al. 1). Using advanced technologies, these models would address discrete mechanistic hypotheses, which could then be evaluated in a human testing phase with adaptive paradigms to validate clinical pathophysiology. However, such successful research would rely on the rigor and quality of large datasets from multiple centers with investigators adopting a coordinated and standardized methodology.
