A NEW LOOK AT THE LEUKOCYTE DIFFERENTIAL The examination of blood smears processed with cytochemical stains has been used as a fundamental diagnostic tool for decades. However, manual differential counts are time and labor intensive, provide particularly poor enumeration of infrequent populations due to the relatively small number of cells evaluated, suffer from nonuniform distribution of cells on the slide, and can misclassify cells having unusual morphology. Automated blood cell counters became practical tools midway through the last century, and currently, the combination of the complete blood cell count with white cell differential is likely among the most requested clinical test. An attempt at improving this analysis with an immunophenotypic flow-based technique has been the subject of a previous report in this journal (1). In this issue, the paper by Roussel et al. (2) is related to prospective new ways to acquire more information from the white blood cell differential. That is, it is an attempt by these authors to further extract information from a flow cytometricbased white cell differential that would identify cases of infection, sepsis and/or septic shock. Rather than the seven parameters currently used in a WBC differential, 50 parameters provided by the reported instrumentation were analyzed by hierarchical clustering in a multidimensional manner from 459 patients admitted to an intensive care unit. Their results are intriguing in their capacity to diagnose cases of infection, sepsis, and septic shock, and underscore the consideration of an extended WBC differential that may provide additional important markers. The contributing and relevant role of dendritic cells in sepsis is discussed in this study and has also been examined in other published papers (3). It would be additionally intriguing to extend and apply these methods to examine cerebral spinal fluid (4) and ascertain further how the use of a flow cytometric-based WBC be more integrated into routine hematology laboratory workflow.
