Anti-citrullinated peptide antibodies (ACPA) are specific to rheumatoid arthritis (RA) and associated with disease severity. The correlation between changes in anti-cyclic citrullinated peptide 2 (CCP-2) test results and disease activity, however, has not been clearly defined.[1-3] Antibodies to glucose-6-phosphate isomerase are pathogenic in mice,[4] we recently reported that anti-citrullinated glucose-6-phosphate isomerase peptide (CCG)-2 and CCG-7 antibodies are specific biomarkers of RA and are decreased by using a TNF antagonist with an associated reduction in disease activity; however, anti-citrullinated alpha enolase peptide (CEP)-1 antibodies are not decreased.[5] Several ACPAs appear to be mixed and cross reactive, but fluctuation of each ACPA with different biologic treatments has not been reported.[6] Anti-CCP2 antibodies have been shown to be deposited on the surface of cultured osteoclasts,[7] and citrulline has been reported to be expressed in the RA synovium.[8] However, it is not clear where within the synovium each ACPA interacts. In this study, we investigated the fluctuation of several ACPAs by three kinds of biologic treatments (infliximab, tocilizumab and abatacept) and studied their deposition to the synovium in RA patients.After informed consent was obtained, plasma samples were obtained from 132 ACPA-positive RA patients. The patients had been diagnosed by rheumatologists at the University of Tsukuba Hospital according to 2010 Rheumatoid arthritis classification criteria.[9] Synovium samples were also collected from ACPA-positive RA patients. ACPAs were measured by ELISA using previously reported protocols.[4] Three cyclic citrullinated peptides were used as antigens: CCG-2, CCG-7 and CEP-1.
