Although the association between nonsteroidal antiinflammatory drugs (NSAIDS) and gastrointestinal (GI) symptoms has been known for many years, the importance of such adverse effects has been subject to debate. Rheumatologists and general practitioners see NSAIDs as an extremely valuable and generally well tolerated kind of drug for first line use in arthritis and musculoskeletal disorders. Gastroenterologists and surgeons insist that they are dangerous irritants of the gastrointestinal mucosa and potentially lethal. These conflicting positions are primarilly based on personal experiences rather than on careful analysis of epidemiological reports. Therefore, the following overview will briefly discuss some general epidemiological aspects of peptic ulcer disease and NSAID-induced gastropathy, then describe the most important data and observations of population and clinical epidemiology and finally point out the need for future research as well as new perspectives.
