We read with great interest the recent report by Schmidt et al, in which they described the proinflammatory role of tumor necrosis factor receptor type I (TNFRI) in the pathogenesis of rheumatoid arthritis (RA)(1). This study highlights that TNFRI signaling is the dominant pathway leading to proinflammatory cytokine and chemokine production in ex vivocultured synovial membrane mononuclear cells (MNCs). Selective blockade of TNFRI is suggested as a potentially therapeutic approach in RA that might be superior to TNF ligand blockade, because it preserves beneficial TNFRII signaling.
