Li et al. demonstrate the feasibility of rapid, high-resolution, near whole-brain 3D MR spectroscopic imaging in acute stroke within real-world clinical environments. Metabolic maps obtained at 3 mm spatial resolution show that the lactate signal can distinguish between areas of infarction, penumbra and oligaemia.
