Following birth, the heart undergoes profound struc-tural and functional remodeling marked by metabolic and contractile maturation of cardiomyocytes. 1 Metabolic maturation to maximize ATP synthesis is triggered by the induction of genes that promote fatty acid metabolism, oxidative phosphorylation, and production of mitochondria, with concomitant downregulation of genes that favor glycolysis. 2 At the level of myofibrils, fetal-to-adult isoform switching of multiple components of the cardiac sarcomere sets the stage for efficient mechanical performance and enables the heart to react to exogenous signals. This is exemplified by the transition from expression of fetal slow skeletal TNNI1 (troponin I) to adult cTnI (cardiac troponin I, TNNI3), 3 which contains an aminoterminal extension that harbors phospho-acceptor sites that govern responsiveness of myofibrils to adrenergic receptor signaling. Understanding the molecular mechanisms that control cardiomyocyte maturation has important implications for cardiac regenerative therapy, the treatment of congenital heart disease and acquired heart failure, which is associated with fetal gene activation, as well as experimentation using human induced pluripotent stem cell-derived cardiomyocytes (hiPSC-CMs), which are often maligned for their immature state. 1, 4
