The evaluation of macular function can be important in many ways in studying macular disease, over and above the simple evaluation of visual acuity. Visual function testing can help to distinguish between macular disease and disease of the optic nerves and higher pathways. It can help to quantify the degree of abnormality present and can measure the progression of disease over time. It can distinguish between diffuse retinal disease and localized macular disease and between disease specific to the cones and nonspecific loss of function. Ongoing efforts seek to use visual function testing to detect risk factors for developing disease, particularly in the fellow eye of a patient with unilateral disease. In this chapter, the discussion of macular evaluation is divided into three sections: clinical electrophysiological evaluation, subjective visualfunction testing (visual psychophysics, including visual acuity, visual-field testing, color-vision testing, retinal sensitivity, contrast sensitivity, dark adaptation, photostress, and reading), and imaging. The goal of the chapter is to acquaint the reader with some of the techniques that are available and what questions may be answered by testing with them. It focuses on the features of each form of testing that are of particular interest in macular disease. Giving a comprehensive review is impossible in the allotted space, so selected pertinent references are given, and readers are encouraged to explore the described techniques further.
