Successful clinical application of an automatic ventricular defibrillator was first reported within the past year? This fully implantable device possesses the capability of continuously monitoring cardiac rhythm, recognizing ventricular fibrillation and ventricular tachycardias characterized by sinusoidal waveform, and delivering corrective defibrillatory discharges when indicated. The principal goal of the automatic implanted defibrillator is to protect highrisk patients from sudden cardiac death by providing such individuals with a self-contained diagnostictherapeutic system able to correct malignant ventricular tachyarrhythmias without medical personnel or additional equipment.~, 3 Since the first implants, 1, 4 clinical evaluations of this new therapeutic modality have continued at The Johns Hopkins University Medical Center. These pilot studies have been designed for the following purposes:(1) to validate information obtained during the preclinical testing program in regard to the functional performance, reliability, and safety of the new device 5. 6 (2) to evaluate performance data concerning the short-and longterm effectiveness of its utilization in patients; and (3) to identify and correct potential difficulties prior to the widespread availability of the defibrillator for broader use. In this report, we present the total clinical experience to date of 16 patients treated with implantation of the automatic defibrillator.
