A Cascade of Care Model for Suicide Prevention.

Suicide remains the second leading cause of death for youth ages 15–19 years in the US From 2011 to 2018, suicide rates in the US increased, while rates in other world regions decreased. 1 In 2019–2020 rates continued to climb in several racial and ethnic minority population groups and among younger age groups. 2 The magnitude of suicide in the US and related disparities underlines the need for a response that specifically focuses on addressing inequities including implementation gaps in delivery and sustainment of best practices to all of those who could benefit. Since 2001, the US has had a national strategy to address suicide, yet more than 20 years later, it has failed to accomplish all its goals and did not specifically focus on ending suicide-related health disparities. In this paper, an alternative model, a suicide-specific Cascade of Care model, is proposed, to guide a more equitable, inclusive, and scalable national suicideprevention response.Building on decades of work to drive down HIV morbidity and mortality, the field leveraged the HIV Cascade of Care to monitor goals and more recently, work towards dismantling disparities in HIV care. Similar models have now been developed for other chronic medical conditions. 3, 4 Cascade of Care (COC) models broadly aim to explain the care and services people with specific health problems need to increase the probability of a favorable outcome. These COC models may be beneficial compared to other similar public health models, as they can be used to track both patient-level progress towards their health goals and assess population-level care outcomes. For example, in an HIV COC model, an individual patient can be tracked from diagnosis to viral suppression. At the policy level, the United Nations established the goal of successfully diagnosing 95% of all HIV-infected persons worldwide, engaging 95% of patients in treatment;
