Until recently, the subject of ‘non-specific’ chronic gastritis was straightforward and noncontroversial. A simple classification based on the depth of mucosal infiltration by chronic inflammatory cells has generally sufficed for diagnostic purposes.’ Division into chronic superficial and chronic atrophic gastritis has been further refined to take account of the presence and severity of acute inflammatory activity, intestinal metaplasia, and glandular atrophy,* but these features are of little or no clinical relevance in routine practice. Previous pathogenetic classifications divide chronic gastritis into two main types. One (Type A), the autoimmune gastritis which leads to pernicious anaemia, mainly affects body mucosa and is associated with hypochlorhydria and the presence in the serum of anti-parietal cell an t ib~d ies .~ The other (Type B gastritis) is found predominantly in the antrum and is associated with hyperchlorhydria and peptic ulceration. Thus far, this category has been considered ‘idiopathic’ although irritants both exogenous, such as hot drinks and spices, and endogenous, like bile reflux, are suggested causes in current textbooks of pathology. A third type, somewhat confusingly designated AB, has been distinguished more recently4 as one that exhibits patchy gastritis in both antral and body mucosa and is associated with a higher incidence of gastric ulcer, dysplasia, and gastric cancer. It must be emphasized that these previously described pathogenetic ‘types’ cannot be distinguished on histological grounds.
