Allergic rhinitis (AR), many presentations of chronic rhinosinusitis (CRS), and most presentations of CRS with nasal polyposis (NP) are recognized as type 2 (T2) inflammatory diseases. These T2high inflammatory conditions are characterized by infiltration with eosinophils, basophils, mast cells, and other inflammatory cells and are recognized by the robust expression of cytokines associated with Th2 effector cells, specifically the interleukins (IL) IL-4, IL-5, and IL-13. The term T2high is now the preferred term for this inflammatory phenotype, in recognition of the numerous additional sources of Th2 effector cytokines such as innate lymphoid 2 cells (ILC2s), eosinophils, mast cells, and numerous others. Although not specifically developed for T2 diseases of the upper airway, recognition of the roles of these T2 cytokines in driving the presence and severity of these conditions has led to the exploration of biologics that were developed for other T2 conditions (atopic dermatitis (AD) and asthma) as therapeutic options. This chapter will review the use of biologics as potential interventions in allergic rhinitis, chronic rhinosinusitis, and nasal polyposis.
