Gestational trophoblastic disease is a term commonly applied to a spectrum of interrelated diseases originating from the placental trophoblast1. These histologically distinct disease entities include complete, partial and invasive moles, placental site trophoblastic tumors and choriocarcinomas. With the advent of a sensitive assay for human chorionic gonadotropin (hCG) for monitoring gestational trophoblastic disease and new approaches to chemotherapy, most women diagnosed as having gestational trophoblastic disease are followed up and can be treated before the development of severe complications. Estimates of the incidence of the various forms of gestational trophoblastic disease vary, mainly because few countries have registries, and complete and partial moles have often been treated as a single entity in epidemiological studies. Furthermore, complete or partial hydatidiform moles presenting as miscarriages are likely to escape detection. Indeed, the vast majority of complete and partial moles abort spontaneously during the first trimester of pregnancy and the incidence of molar pregnancies has been estimated to be 1 per 41 miscarriages2.Pulmonary complications due to trophoblastic embolization are frequently observed following the evacuation of a molar pregnancy3 and the prognosis for these patients depends upon the severity of the symptoms. Early diagnosis reduces the risk of severe complications and, in particular, of respiratory failure. Ultrasonography has replaced all other means for early screening and for establishing the differential diagnosis of molar pregnancies in utero4. The current review was undertaken to evaluate the role of ultrasound and Doppler examination in the diagnosis, follow-up and management of patients with gestational trophoblastic disease.
