To the Editor—Mwimanzi et al recently reported that the third dose of a coronavirus disease 2019 (COVID-19) vaccine boosted Omicron-specific neutralization in older individuals, and they noted that the number of chronic health conditions was the strongest factor of weaker humoral responses, indicating the immune benefits of a third vaccine dose [1]. Older adults and individuals with comorbidities are at increased risk of lethal COVID-19 disease caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection [2, 3]. SARS-CoV-2 Omicron variants, the transmission of which is greater than that of previous variants, have appeared with several mutations, especially in the spike protein; some of the mutations are important for immune escape, and Omicron variants have thus spread rapidly and become an important, worldwide problem [4].It is, therefore, important to determine whether a third vaccine dose affects disease severity among COVID-19 patients infected with an Omicron variant. We investigated the clinical features of 23 patients whose COVID-19 became severe and who were admitted to the intensive care unit at Hyogo Prefectural Kakogawa Medical Center (Kakogawa, Hyogo, Japan) after they were infected with the Omicron BA. 1 or BA. 2 variant during the 3-month period of January–March 2022 (Table 1).
