悪性リンパ腫と鑑別が必要な良性病変や境界病変がいくつか知られており、その中に当部署が疾患概念を提唱した良性natural killer(NK)細胞増殖性疾患であるリンパ腫様胃症/NK細胞腸症(Lymphomatoid gastropathy:LyGa/NK-cell enteropathy:NKCE)がある。非常に希少な疾患で実態の把握はされておらず、明確な診断基準は現在存在しないが2019年にSTAT5活性とJAK3遺伝子の異常が報告され腫瘍性の根拠が示された。しかし、JAK3遺伝子異常は30%にしかみられず、他のSTAT5活性に関連した遺伝子異常があると考えられる。本研究の目的は症例数を集め本邦における本疾患の実態把握およびSTAT5活性化の関連する遺伝子の検索をおこない、本疾患の原因遺伝子を特定し診断基準を明確にすることである。昨年度１年間で他施設病理部から1症例のLyGa/NKCEが集まり、患者年齢、性別などの患者背景および腹痛の有無や採血結果などの臨床情報の集積をおこなった。さらに凍結検体も保存されている症例で、現時点では免疫染色まで行い診断を確定している段階であるが、今後は遺伝子検索を行う予定である。実態把握および原因遺伝子異常の検索を行うためにも十分な症例数を確保することが重要であるが他施設病理部への問い合わせや当院へのセカンドオピニオン症例などだけでは、まだまだ症例数が少ないのが現状である。上部消化管内視鏡像が特徴的であるため消化管内視鏡医へ典型的な内視鏡像の経験の有無、そのような経験がある場合はさらに生検結果でNK細胞増殖であったかを他施設に問い合わせする予定である。
