我々のグループは、日本で最大規模の甲状腺専門病院である隈病院と共同で、家族性発症の多結節性甲状腺腫 (multinodular goiter, MNG）について、生殖細胞系列の遺伝子型に関するデータを集積しており、最近、遺伝子パネル検査により、予想外にサイログロブリン遺伝子(TG) のヘテロ接合性バリアントの頻度が高い(約33%) ことを見出した。一般的にTG 異常症はバイアレリック（ホモ接合型あるいは複合ヘテロ接合型）変異で“甲状腺腫を伴う甲状腺機能低下症”を発症し、TG のモノアレリック（ヘテロ接合型）変異の病的意義についての報告はなく、変異部位の違いによる病的効果の多様性も想定される。そこで本研究では、TG バリアントを伴う家族性多結節性甲状腺腫の成因の解明を、甲状腺組織内の2次的な腫瘍関連遺伝子異常の検索及び細胞実験による当該バリアントの病原性評価により試みることとした。さらに散発性MNG における TG バリアントの関与についても検討する予定とした。今年度も、予備実験で設定した遺伝子パネル検査を利用し、生殖細胞系列遺伝子バリアントについて、当初の予定を大きく上回る86検体の家族性甲状腺腫症例の解析を行った。さらに、体細胞バリアント検索のための遺伝子パネル検査のパフォーマンスについての検討を行った。引き続き、生殖細胞バリアントの遺伝子型・表現型の関係について、レトロスペクティブに解析を行う予定である。
