京都市左京区認知症初期集中支援チームのチーム員である西村伊三男（研究分担者）、川瀬美奈子（研究協力者）とカンファレンスを8回行った。また軽度行動障害(MBI)を認める高齢者に対して京都市左京区と伏見区の認知症初期集中支援チームが介入し、京都府立医科大学附属病院に紹介していただき、定期的な外来通院に繋がった2症例について、学会で発表し、老年精神医学雑誌に掲載された。MBIの画像研究は多く行われているが、システマティックレビューはなかったため行った。MBIの画像研究では海馬、海馬傍回、嗅内皮質、側頭葉の萎縮やネットワークの機能異常がMBIに関与していることが示唆されたが、これらの脳領域は認知症の初期で認められる変化であり、MBIは認知症の病理学的変化を背景に生じていることが示唆された。つまり、MBIは認知症のリスクファクターというよりも前駆症状であることが示唆された。孤独感がMBIに関係していることを前年度に学会発表したが、次に孤独感と関係している脳領域を調べた。その結果、孤独感は視床、海馬、嗅内皮質の萎縮と関与していた。孤独感は認知症のリスクファクターとも言われており、アルツハイマー型認知症(AD)で初期から萎縮する海馬、嗅内皮質と孤独感が関係していたという結果は、孤独感と認知症の関係を支持していた。60歳以降に発症する精神病（幻覚、妄想）であるvery late-onset schizophrenia-like psychosis (VLOSLP: 最遅発性統合失調症様精神病)は認知症の前駆症状の可能性が示唆されている。また松果体体積はADで減少しているとも言われている。そこで、VLOSLPと松果体体積との関係について調べたところ、VLOSLPでも松果体体積が減少していた。そのため、VLOSLPはADの前駆症状の可能性もあることが示唆された。
