当施設は九州における成人先天性心疾患診療の中心的施設であり、フォンタン手術後フォローアップ患者数は右肩上がりに増加している。フォンタン循環は高中心静脈圧、低心拍出を特徴とする。多くの症例は比較的安定した血行動態を示しているが、一部には心拍出量が増加（一般集団の定義から考えると正常心拍出量）して、Fialed Fontanを呈している患者が存在する。この患者群では体血管抵抗が低い傾向にある。一方で、肺血管抵抗は上昇していない。またアンモニアや総胆汁酸の上昇がみられており、フォンタン循環による肝障害（肝臓の線維化）から本来肝臓で代謝されるべき血管拡張物質が全身へ播種し血管拡張へ傾いていると考えられた。ACE阻害薬などの血管拡張剤や肺血管拡張剤を投与されている症例は多いが、これらの薬剤がこのような病態を修飾している可能性も示唆された。また、チアノーゼが出現している症例では肺動静脈婁の存在も懸念される。門脈圧亢進を伴う場合には脾機能亢進に伴う血球減少なども含めて種々の影響が出てくる。門脈体循環シャントが存在する場合は、先天性なのか、二次的かは判断に苦慮するため、塞栓をするかを判断するのは非常に困難であることが示唆された。アンモニアによるスクリーニングを継続し、今後も症例数の集積を継続する。また、血管拡張剤や血管収縮剤の効果についても後方視的にではあるが、血行動態への影響を検討していきたいと考えている。
