患者重症度に合わせて医療施設を適切にご案内する方法の一つとして、重症患者の転院に関する解析が有用と考え、当院に心不全加療について相談のあった重症心不全症例の転院について解析した。補助人工心臓や心臓移植検討目的に当院に転院した症例について、近距離の医療施設からの転院と長距離の医療施設からの転院に分け、転院後の転機について確認した。とりあげたアウトカムとしては転院後のカテコラミン増量、機械的補助の追加、腎機能の増悪について、心不全悪化ととらえカウントした。結果として長距離の医療施設からの転院症例については、転院後の心不全悪化のアウトカムが有意に増加した。このことより、やはり長距離の転院についてはリスクが高くなり、そのような転院の前により近隣の中堅病院に転院していく方法がより適切と考えられた。以前の同様の検討で強心薬投与を経験した心不全患者については補助人工心臓・心臓移植あるいは死亡に至るリスクが高いことが確認されており、今回の検討を合わせて考え、そのような既往がある患者に関しては早期の段階でより中心的、専門的医療施設に段階的に紹介することが望ましいと考えられた。
