本研究の目的は、非乳頭部十二指腸腺腫・十二指腸癌におけるERBB受容体ファミリーの遺伝子変異・遺伝子発現を包括的に評価することにより、発癌への関与を明らかにし、治療標的となり得るかどうかを検討することである。さらに、十二指腸癌診療におけるERBB受容体ファミリーのバイオマーカーとしての有用性を明らかにすることである。研究協力施設において内視鏡的もしくは外科的に切除され、保存されている非乳頭部十二指腸腺腫、粘膜内癌、浸潤癌の計 71病変を対象とした。これらの病変は、複数の消化管専門病理医によって病理学的に再評価された。そして、早期病変は、粘液形質の免疫組織化学（CD10, MUC2, MUC5AC, MUC6, PG-I）によって胃型と小腸型への分類が行われた。結果として、上記の病変は小腸型腺腫 32病変、幽門腺腺腫 6病変、鋸歯状腺腫 1病変、小腸型粘膜内癌 12病変、胃型粘膜内癌 7病変、浸潤癌 13病変に分類された。全病変に関して、EGFR, HER2 (ERBB2), ERBB3, ERBB4の免疫組織化学を施行した。さらにHER2の免疫組織化学で2+以上であった症例のみ、HER2 dual color in situ Hybridization (DISH)での評価を行った。免疫組織化学に並行して、ホルマリン固定パラフィン包埋切片からDNA抽出をおこなった。DNAのクオリティチェックを終了し、ERBB2, ERBB3, ERBB4の遺伝子変異とコピー数変化（遺伝子増幅）の解析を行った。

The objective of this study was to evaluate the role of ERBB receptor in non-papillary duodenal adenomas and duodenal cancer, including the development of ERBB receptors, the relationship between cancer development and diagnosis, and the identification of therapeutic targets. The ERBB receptor is useful in the diagnosis and treatment of duodenal cancer. The study was conducted in conjunction with endoscopic surgery for 71 cases of nonpapillary duodenal adenomas, intramucosal carcinomas, and invasive carcinomas. This article reviews the pathology of digestive tract diseases. Immunohistochemistry (CD10, MUC2, MUC5AC, MUC6, PG-I) was used to classify gastric and small intestinal types. Results: 32 cases of small intestinal adenomas, 6 cases of pyloric adenomas, 1 case of serous adenomas, 12 cases of small intestinal intramucosal carcinomas, 7 cases of gastric intramucosal carcinomas, and 13 cases of invasive carcinomas were classified.
