本研究では、申請者らが実施している難治性脳腫瘍（悪性神経膠腫、治療抵抗性の神経鞘腫・髄膜腫・脊索腫等）新生血管を治療標的としたペプチドワクチン療法臨床試験被験者検体を用いて、抗腫瘍免疫ネットワークにおける正と負の調節細胞・分子や腫瘍血管新生・浸潤性関連因子について解析し、同治療の臨床的有効性との関連性を評価する。この結果に基づいて、同がんワクチン免疫療法の治療有効性を予測し得る因子を同定、がん免疫病態の個体差を解明し、同療法に対する反応性・抵抗性の新たな評価法を開発することを目指す。2022年度は、(1) 同ペプチドワクチン療法臨床試験被験者検体の収集と保存と、(2) 標準治療が施された患者検体、および同臨床試験患者検体を用いた脳腫瘍血管新生・浸潤性関連因子、および免疫応答関連細胞・因子についての解析を計画していた。(1) 現在進行中の「初発悪性神経膠腫に対するがんペプチドワクチン療法の第I/II相臨床試験」1症例、「再発・進行性難治性脳腫瘍に対するVEGFR1/2ペプチドワクチンの第Ⅰ/Ⅱ相臨床試験」2例（脊索腫1例、孤在線維性腫瘍1例）の被験者の検体および臨床情報を新たに収集した。(2) 標準治療が施された難治性脳腫瘍患者検体を用いて、脳腫瘍血管新生性関連因子であり、抗腫瘍免疫応答抑制性因子であるVEGF/VEGFRの腫瘍組織発現や、腫瘍細胞の増殖・浸潤能を高めている特定の分泌型microRNAの患者血清での検出等を実行し、新規臨床試験被験者検体で同項目を評価する際のコントロール群としての情報を蓄積した。さらに進行中の臨床試験被験者検体を用いて、同ワクチン療法が標的としている抗原（VEGFR）を発現している細胞に対して反応性をもつ細胞傷害性T細胞（CTL）の頻度を解析したところ、同ワクチンによって被験者体内に抗原特異的CTLが高頻度に誘導されることが明らかになった。

This study was conducted in response to the applicant's request for treatment of refractory cancer. Clinical trials of neovascularization therapy targets (neurogliomas, treatment-resistant neurofibromas, fibromas, chordomas, etc.). Clinical trials of anti-tumor immunotherapy. The results of this study are based on the identification of factors for predicting the effectiveness of immunotherapy, the identification of individual differences in immunopathology, and the development of new methods for evaluating resistance. In 2022,(1) the collection and preservation of patient samples from clinical trials of the same type of therapy, and (2) the analysis of tumor angiogenesis, infiltration-related factors, and immune-related cellular factors in patients with standard therapy were planned. (1)We are now conducting a new collection of patient's clinical information on "Phase I/II clinical trials of VEGFR therapy for incipient neuroma" in 1 case and "Phase I/II clinical trials of VEGFR1/2 therapy for relapsing and progressive refractory neuroma" in 2 cases (1 chordoma and 1 isolated linear neuroma). (2)Standard therapy is used in patients with refractory tumors, tumor angiogenesis related factors, anti-tumor immune response inhibitory factors, VEGF/VEGFR tumor tissue development, tumor cell proliferation and infiltration, specific secretory microRNA detection in patients 'sera, and new clinical trials. Clinical trials are currently underway to determine the frequency of antigen-specific CTL induction in patients with antigen-specific CTL.
