プラチナ抵抗性再発転移頭頸部癌に対するニボルマブの投与が、腫瘍関連B細胞にどのような影響を与えるのか明らかにするために、本年度はニボルマブ投与前後の末梢血中サイトカインの変動について解析を行った。ニボルマブが著効する患者は炎症性サイトカインが減少し、増悪する患者は抑制性サイトカインが増加する傾向を認めた。プラチナ抵抗性再発転移頭頸部癌に対するニボルマブ（免疫チェックポイント分子PD-1を阻害する抗体製剤）の治療効果は、これまでの治療と比して劇的であるにもかかわらず、その奏功率は20ー30％程度に留まる。これらの現象にB細胞が関与しているのかは未だ多くの点が不明である。これまでの研究において、腫瘍抗原に対する抗体の産生はニボルマブによるPFSの延長に関して負の相関がある可能性が示唆されている。本年度は、昨年度に引き続き、被験者の登録並びに経時的採血を継続し、併せて臨床経過の評価も行ってきた。さらに末梢血において抗体以外の変動因子を確認するためニボルマブ投与前後の末梢血サイトカインの変動について解析を行った。調べたサイトカインのうち抑制性サイトカインの一種であるインターロイキン-10（IL-10）は増悪症例（PD）において投与前よりも投与後に増加する傾向を示した。一方、炎症性サイトカインの一種である腫瘍壊死因子（TNF-α）は部分著効症例（PR）において投与前よりも投与後に減少する傾向を示した。また、多機能リン酸化糖タンパク質の一種であるオステオポンチン（OPN）もまた、部分著効症例（PR）において投与前よりも投与後に減少する傾向を示した。
