頭頸部がんの個別化医療の実用化に向けたセンチネルリンパ節ナビゲーション手(SNNS）に関わる国内多施設共同研究が平成21年より実施されている。乳がんにおいては微小転移の基準があり、これらに応じた診療ガイドラインが定められているのに対し、頭頸部がんは基準が存在せず、リンパ節転移の有無のみの一律の方針で診療が行われている。 本研究は先行研究を対象に転移リンパ節の状況を後ろ向きに確認して予後を比較解析し、センチネルリンパ節（Sentinel Node,SN）が微小リンパ節転移（以下、微小転移）とみなした場合に頸部郭清の省略が可能かを後ろ向きに検討し、SNNSで同定した微小転移例の頸部郭清の省略が可能であるかどうかの比較検証する前向き無作為比較試験を準備することを目的とし、頭頸部がんに対するSNNSの診療ガイドラインの作成することを到達目標とする。2021年度に構築したデータベースから得られた臨床情報を後ろ向きに照合し予後と転移病巣の径との関係を調査しSNの下流に後発を含むリンパ節転移が出現するリンパ節径を算出し、微小転移と診断するカットオフ値案を作成し国内の学術会議で報告し、参加者から得た意見を参考に2022年は論文を作成し投稿した。2023年度は論文化ののち、次の研究に継承するため、疫学有識者と協議しながらSNが微小転移であった際に、頸部郭清を省略するか、画一的な頸部郭清術を行うかとの治療法の比較・検証を行う前向き比較試験をデザインし今後の研究デザインを構築する。
