（はじめに）頭頸部癌における頸部郭清術は重要な治療手段である。100年以上前から施行され、根治的に頸部組織を一塊摘出することで根治性を狙う概念である。しかしながら現在腫瘍免疫の概念が確立し、免疫チェックポイント阻害剤が頭頸部癌において標準治療となっている。頸部には抗腫瘍免疫に重要且つ必要な頭頸部所属リンパ節が存在する。もし頸部転移症例の頸部リンパ節に抗腫瘍免疫能が存在するならば、従来の頸部郭清術概念を考え直す時期に来ている。HPV関連頭頸部癌で実験系を考案した。（対象と方法）頭頸部癌p16陽性HPV関連腫瘍4例に、原発切除と頸部郭清術を施行した。原発腫瘍、転移リンパ節、転移なしリンパ節（近位と遠位）、末梢血からリンパ球を抽出した。これらをHPV E6, E7の抗原で14日間刺激し、その後E6, E7特異的ペプチドにて特異的T細胞増殖が見られるかどうか、インターフェロンγ、TNF-α刺激でフローサイトメトリーで確認した。HPV発現はRNAスコープ発現で確認した。（結果）全4例でHPV16発現が確認された。p16免疫染色陽性は、HPV16感染と関連していることが示された。抗腫瘍免疫に関しては１例において、 E6, E7の特異的CD8陽性T細胞が転移リンパ節に確認された。さらに、近位の転移なしリンパ節おいても確認された。 一方、E6, E7特異的CD4陽性T細胞は全ての症例で確認されなかった。（考察）本研究により、HPV関連頭頸部癌の頸部転移症例において、癌の頸部転移が存在するのにも係わらず、その頸部転移リンパ節では抗腫瘍免疫が働いていることが示された。更にその近傍の転移なしリンパ節においても同様に抗腫瘍免疫が働いていることも示された。外科腫瘍免疫学が確立される現在、従来の頸部郭清術の概念を変えていく事も必要とされる事が示された。頸部転移リンパ節レパートア解析も行っている。
