これまで不可逆的とされていた糖尿病性感覚神経障害の中から足根管症候群と診断され、改善可能な症例を抽出し、鏡視下手術により神経の絞扼を改善することができれば足部の潰瘍発生予防や大切断に至る症例の回避につながる新たな治療戦略が生まれる。本研究の目的は、糖尿病患者の中でも足の異常知覚、知覚鈍麻などの感覚神経障害をきたしている症例から足根管での絞扼性神経症状を原因とした神経障害を抽出し、足根管レベルでの絞扼性神経障害に対する鏡視下神経除圧術の有効性を検証することである。初年度2021年と本年度2022年度は、その中間調査として実臨床を各学術集会で発表した。本研究は、前向き・介入研究で行う。糖尿病合併症重症化予防のフットケアを施行している患者を対象に、臨床的神経評価（足部モノフィラメントテスト、振動覚検査など）、神経伝導速度（脛骨神経）などの術前検査を行い、足根管症状群と診断された症例に対して小切開による鏡視下神経除圧術を行う。対象者の選択・除外基準は、1.糖尿病の診断を受け、自覚症状として足部の感覚異常（ムズムズ、灼熱、無知覚など）を訴える。2.足関節内果部の足根管直上にTinel様兆候がある3.術前に下肢の脛骨神経伝導速度で遅延が認められる。4.血糖コントロールの投薬治療を受けており、血糖のコントロールが良好である。除外基準として、重症下肢虚血を除外する（ABI 0.4 以下）。今後、症例数を３年間で30例とし、経過観察も含めた研究期間を５年とする。研究期間の３年間で術後２年の中期結果を報告し、リスク患者の潰瘍発生予防率を出す。また並行してどの程度（発病からの期間）の糖尿病性感覚神経障害患者が足根管症候群と診断され、感覚が改善可能であるのかを検討する。

This is an irreversible disease that affects the development of diabetic ulcers, including root canal syndrome, diagnosis, improvement of possible cases, microscopic surgery, improvement of nerve management, prevention of major foot amputations, avoidance of symptoms, and new treatments. The purpose of this study was to investigate the causes of abnormal cognitive and sensory neurologic impairment in the middle and lower extremities of diabetic patients, and to identify the effective mechanisms of neurologic impairment in the root canal of the foot. In 2021 and 2022, we conducted an intermediate survey and conducted clinical research. This research is backward, forward, and intervention research. Preoperative examination of diabetic complications, prevention of diabetic complications, clinical neurological evaluation (foot, vibration), neurological conduction velocity (tibial), diagnosis of root canal symptoms, and diagnosis of diabetic complications. 1. Diagnosis of diabetes mellitus, symptoms of diabetes mellitus and abnormal sensation of feet (such as fever, fever and ignorance) 2. 3. Preoperative delay of tibial nerve conduction velocity in lower limbs 4. Blood sugar levels are good for treatment. Except for baseline and severe lower limb deficiency (ABI 0.4 or less). In the future, the number of cases will be 30 in 3 years, and the investigation period will be 5 years. Interim results were reported during the 3-year study period and 2 years after surgery, and the incidence of ulcer development in patients was reported. The diagnosis of foot root canal syndrome and the possibility of sensory improvement are discussed in patients with diabetic neuropathy who have increased the severity (duration) of the disease.
