口腔癌個別化治療を目指した頸部リンパ節転移節外浸潤における腫瘍間質反応の解明

项目摘要

1. 腫瘍間質の予後不良因子に関する臨床研究口腔扁平上皮癌頸部リンパ節転移巣節外浸潤（ENE）を認める症例を対象に、間質繊維化反応（DR）を含めた予後因子の同定を行った。対象および方法:当科にて口腔扁平上皮癌で一次治療を行なった604症例のうち、病理組織学的にENEを有する59 症例を対象とした。年齢、性別、原発部位、T分類、頸部リンパ節転移時期、ENEを有する転移リンパ節の最遠位レベル、個数、原発巣の分化度、リンパ管侵襲、脈管侵襲、神経周囲浸潤の有無、浸潤様式(YK分類)、リンパ節および原発巣のDRの有無と治療成績(生存率)について検討を行った。結果:ENEを有する転移リンパ節の発現レベルが、I・II・IIIよりIV・Vの遠位レベルの方が5年全生存率(5 year overall survival : 5yOS)と5年疾患特異的生存率(5 year disease specific survival : 5yDSS)いずれも有意に生存率が低い結果であった。また ENEを有する転移リンパ節の個数が、単発より複数個の方が5yDSSが有意に低い結果であった。また、転移リンパ節のDRの有無について検討した結果、5yOSはDR(+)群が54%に対しDR(-)群が83.3%、5yDSSではDR(+)群が64.9%でDR(-)群は 100%でいずれもDR(+)群では生存率が低い結果であった。2．腫瘍微小環境の免疫組織化学的な解析上記対象者59症例の切除標本（転移リンパ節病変）を用い、多重免疫染色を行う。腫瘍微小環境におけるリンパ球系細胞、骨髄系細胞、免疫チェックポイント関連、癌関連線維芽細胞、間質線維化反応、上皮間葉転換などの局在・定量解析を行い、治療効果、予後、生存率などの臨床情報と照らし合わせ、バイオマーカーの特定を行う。

1. Clinical Study on the Relationship between Tumor Stroma and Prognostic Factors in Oral Squamous Cell Carcinoma (OSCC) Methods: 604 cases of squamous cell carcinoma of the oral cavity were treated at one time, and 59 cases of ENE were treated at one time. Age, sex, site of origin, T classification, period of cervical node migration, presence or absence of ENE, number of distal nodes, degree of differentiation of origin, presence or absence of vascular invasion, presence or absence of neurovascular invasion, pattern of invasion (YK classification), presence or absence of DR of origin, and treatment outcome (survival rate) were examined. Results:ENE has a 5-year overall survival (5-year OS) and a 5-year disease-specific survival (5-year DSS). The number of nodes in the ENE is 5yDSS, and the number of nodes in the ENE is 5yDSS. The survival rate was 54% in DR(+) group and 83.3% in DR(-) group. The survival rate was 100% in DR (+) group. 2.
