本研究は「cN0舌癌における予防的頸部郭清術の前向き観察研究（END-TC study）」として科学研究費（基盤研究C）の補助を得て、2018年度より開始された。症例登録後に予防郭清をランダム化せずに根治治療を施行し，その後統一した経過観察を行うという実臨床に最も即した形の前向き観察研究を行うことにより，早期舌癌に対する予防郭清の明確な適応基準を明らかにすることを目的としている。本研究は2022年に参加登録症例数が600名に達し、登録終了になった。現在、各施設で経過観察とデータ収集を行っている。データー管理も引き続き信州大学が参画する大学病院臨床試験アライアンスが運営する臨床研究支援システム（ACReSS）を用いて行う。結果については研究期間終了後にしか閲覧できないため、登録終了後に解析を行う予定である。本研究の解析段階での最大のポイントは，ランダム化しない形での前向き観察研究であるという点である。それ故，比較する研究群間の症例数の違いや背景因子の違いが大きくなる。そこでベースライン特性を調整するために傾向スコア法で症例のmatchingを行い解析する。また、本試験では多数の症例を扱うため舌癌潜在的頸部転移のバイオマーカーの特定が期待できる。そのため、将来に向けての組織バンク構築を行うとともにバイオマーカーとしてCCND1の発現が有用であるかを明らかにするため検索を行う。また、組織バンクの構築も行う。
