長期間の縦断的疫学研究（久山町研究）によると、握力は骨格筋力を反映する指標であり、認知症や死亡リスクと関連することが報告されており、握力は、全身の健康状態を示す重要な指標として最近注目されている。また我々は、若年成人の握力の低下に顎顔面形態異常や不正咬合および咬合力の低下が関与することを報告し、顎顔面形態やその機能異常が骨格筋力の低下をもたらすことを示唆した。これは、顎顔面形態や咬合が骨格筋力に影響を与えることを示唆した初めての報告であるが、発症機序については不明である。また、先行研究で示した顎顔面形態異常や不正咬合を示す患者は、ブラキシズムや咀嚼筋痛障害を生じやすいことや咀嚼筋痛障害は血液中の炎症性サイトカインが増加することが報告されている。一方、骨格筋力の減少にも、炎症の関与が報告されている。以上より、骨格筋力の低下を引き起こす発症機序として、咀嚼筋痛障害や炎症反応の関与が示唆されるが、これについても不明である。そこで本研究では、顎顔面形態異常や不正咬合、咀嚼筋痛障害、炎症性メディエーターの産生、および骨格筋力との関連を調べて、骨格筋力低下の発症機序を解明することを目的とした。現在、顎顔面形態異常や不正咬合を伴う患者において、咀嚼筋痛障害を有する者を認めるため、何かしらの関連があると予想される。また、咀嚼筋に圧痛を認めるものは、覚醒時の咬筋活動が多く、覚醒時ブラキシズムと関連する可能性が示唆された。また、外科的矯正歯科治療を行う患者の方がボディイメージに不満が高いことが示唆された。今後、顎顔面形態異常や不正咬合、咀嚼筋痛障害、骨格筋力および心理についても分析し、これらの関連を解明するとともに被検者数を増やして調査を継続する。
