近年、疫学研究の進歩とともにエビデンスを構築するだけでなく、エビデンスの実践によるアウトカム評価を行う、医療の質研究が行われている。医療の質の測定により実臨床で行われている医療のみならず臨床への還元の可視化が期待できる。医療の質の評価指標であるQuality Indicator（QI）は、脳卒中領域においても適切な医療提供体制の再構築のために活用されているが、医療の進化とともにその質の評価も変化することが予想される。そのため、持続的に適宜測定可能な仕組みが欠かせない。われわれは2007年より急性期脳卒中の登録事業を行い、脳卒中診療におけるエビデンスの構築に寄与してきた。本研究では、大規模脳卒中登録研究を電子カルテ情報やDPC情報と突合させたデータベースを構築することで、持続的な医療の質の評価とその妥当性の評価を実現させることを目的とし、脳卒中診療における持続的な医療の質の改善とともに、脳卒中診療の最適化をめざしている。大規模脳卒中登録研究である、福岡脳卒中データベース研究は、福岡県下の7つの脳卒中専門医療機関で共同で行っている前向き登録研究である。本年度は、昨年度に引き続き、福岡脳卒中データベースのデータ整備を行った。データベースには急性期脳卒中患者17074名が登録されている。登録患者のデータ収集は、専門の臨床研究コーディネーターにより入力項目を標準化したうえで行い、予後の追跡情報は、脳卒中の予後調査に関して専門的な知識を有する臨床研究コーディネーターにより情報収集している。予後情報については、イベント評価委員会によるイベント判定も行った。現在、用いられているQIの項目についてデータのクレンジングを終了し、解析を開始した。

In recent years, advances in epidemiological research, construction of medical facilities, implementation of medical facilities and evaluation of medical quality research have been made. The quality of medical treatment is measured in clinical practice and expected to be visualized in clinical practice. The Quality Indicator (QI) is intended for use in the reconstruction of medical care delivery systems in stroke areas and for the evolution of medical care. It is suitable for the determination of the possible group of people. Since 2007, the registration of acute stroke has been in operation, and the construction of stroke diagnosis and treatment has been in progress. The purpose of this study is to establish a database of information, DPC information, and emergency response systems for stroke diagnosis and treatment, and to optimize stroke diagnosis and treatment. Large-scale stroke registration study, Fukuoka stroke registration study, Fukuoka Prefecture 7 stroke registration study This year, the company launched a series of new projects in Fukuoka City. 17074 patients with acute stroke were enrolled in the study. Register the patient's data collection, standardize the input items, track the follow-up information, and collect the clinical data collection related to the follow-up investigation of stroke. The following information is provided for the evaluation committee to determine whether the decision is made. Now, the project is finished and the analysis is started.
