项目摘要

腎代替療法は物理的・時間的拘束により患者の苦痛が大きい。治療の意思決定とその後のQOLは、心理社会的問題に強く影響されるが、本邦では慢性腎臓病患者の治療選択に対する心理社会的支援が標準化されていない。本研究の目的は、腎代替療法の意思決定とQOLを支えるための患者と医療者双方向性の心理社会的支援モデルを開発するための基盤的知見を創造することである。2022年度は、データセットの構築を継続し、腎代替療法の治療選択をする段階にある慢性腎臓病患者を対象に公認心理師による心理社会的評価面接と質問紙調査を行った。調査項目は、医療者の情報提供の質と量 / 患者の理解度（共有決定度質問紙：SDM-Q-9 / SDM-Q-Doc）、健康関連QOL（MOS12-Item Short-Form Health Survey）、基礎統計（年齢、性別、仕事など）、診断名、認知機能（MoCA-J）、ヘルスリテラシー (Functional, Communicative, and Critical Health Literacy尺度)、抑うつ（Patient Health Questionnaire）、不安（Generalized Anxiety Disorder）などである。研究成果について、学会で報告をした。次年度以降も、引き続きデータセットの構築を行うとともに、腎代替療法導入時における評価を行い、研究成果の報告をする。本研究に関する有害事象、実施計画書からの逸脱は認められていない。また脱落例も認められておらず、本研究における研究全体の基本的な手順は遵守されていると判断する。

Kidney replacement therapy is a physical and temporal constraint that causes pain and suffering. QOL for rational decision making and psychosocial support for chronic kidney disease in our country The purpose of this study is to create knowledge and support for rational decision making in renal replacement therapy and bi-directional psychosocial support for patients and healers. In 2022, we conducted a survey on the treatment of chronic kidney disease by recognized psychologists. Survey items, quality and quantity of information provided by medical personnel/understanding degree of patients (Common Decision Question Paper: SDM-Q-9 / SDM-Q-Doc), Health Related QOL MOS12-Item Short-Form Health Survey (Age, Gender, Official Status), Diagnostic Name, Cognitive Function (MoCA-J), Category: (Functional, Communicative, and Critical Health Literacy Scale),(Patient Health Questionnaire),(Generalized Anxiety Disorder) Research results are reported and learned. In the next year, we will review the implementation of renal replacement therapy and report on the results of our research. This study is concerned with harmful phenomena and implementation plans. This study focuses on the basic principles of compliance and judgment.
