项目摘要

三重県内の9つの医療施設が参加する三重地域圏統合型医療情報データベース（以下、Mie-LIP DB）を活用したヨード造影剤の安全使用推進にかかる研究である。造影CT施行時にアドレナリンが投与された事例はアナフィラキシー反応を発症した患者である可能性が高いという前提に基づいている。当院を含む3施設において、2017年～2020年の4年間で、造影CTが施行された患者の内、同日にアドレナリンの投与がなされた（造影CT×アドレナリン）事例についてのMie-LIP DBからの抽出作業が完了した。当院の事例においては、医療用データウェアハウスシステム：CLISTA!（以下クリスタ）から得た情報、薬剤副作用収集システムから得た情報、インシデントレポートから得た情報とも比較検討した。Mie-LIP DBからの抽出データでは、造影CT×アドレナリン事例は、当院においては2017年52件、2018年59件、2019年78件、2020年62件であった。また、A病院においては、2017年14件、2018年15件、2019年33件、2020年21件、B病院においては、2017年13件、2018年20件、2019年18件、2020年15件であった。当院におけるクリスタからの抽出による造影CT×アドレナリン事例は、2017年180件、2018年261件、2019年161件、2020年28件（年度単位）であった。また、薬剤副作用収集システムから得た造影剤アレルギー事例をカルテに突合し、アドレナリンが同時に投与された事例を抽出すると2017年0件、2018年5件、2019年5件、2020年3件であった。さらに、インシデントレポートとして、報告された造影剤アレルギー事例は、2017年0件、2018年6件、2019年6件、2020年4件であった。

There were 0 cases in 2017, 5 cases in 2018, 5 cases in 2019 and 3 cases in 2020. The number of cases reported in the report was 0 in 2017, 6 in 2018, 6 in 2019 and 4 in 2020.
