不登校数は、令和3年度24.4万人と前年度から5万人超増加し、中学生全体数の4%超である。不登校前は、身体症状を認めるが80%超は適切な医療享受ができていない。適切な医療享受ができなかった身体症状に早期医療介入し不登校を予防するために、私たちは平成30年から本研究費（課題番号18K09962-0001・21K10365）の助成で、教育と医療が連携して早期医療介入する学校健診システムを開発してきた。方法は、学校で子ども自身が質問紙に回答し、ハイリスクの子どもには校医を受診するように促し、医療機関で身体症状に早期介入するシステムである。2022年度までに7539人を対象に健診を実施し、子ども自身が記入する質問紙と医療機関での早期介入が不登校予防に有用であることを証明した。一方、全国的に本健診を実装するため課題が表在化した。１）ICT（Information and Communication Technology）用新質問紙開発質問紙（QTA30）はICTで使用できないため新質問紙を開発する必要がある。新質問紙の項目は、QTA30 4512名分の蓄積データからハイリスク群に影響する項目を抽出し、加えてQTA30では検出できない子どもの発達特性を検出する項目を追加し、48項目の新質問紙を作成した。２）小児科医以外の校医用マニュアル作成2021年度に校医用のマニュアルを作成し、ハイリスク児の受診先である校医（小児科医）が実用できることを検証できた。今後、全国で本健診を実施するためにハイリスク児の受診先である校医へ依頼するために医師会へ説明したが、内科・整形外科・眼科など小児の診療に不慣れな校医が80%おり、小児医向けの診療マニュアルでは実動できないという意見がでたため、小児科以外の校医向けのマニュアルを作成した。
