周産期死亡症例のピアレビューによる死亡回避を目指した提言策定とその啓発

本研究目的は、県内の全周産期死亡例の「ピアレビュー」により得られた死亡回避の為の「提言」の有効性を検証することである。2022年度は計4回（2022.4/21、5/26、6/23、7/21）にわたり症例検討会を開催し、2020年の県内周産期死亡症例（後期死産21例、新生児死亡11例）を検討した。解析不能例を除くと回避可能と判断された症例はなかった。回避可能と判定された死産症例はなかったが、死産回避のための提言として、妊婦の胎動減少時の速やかな受診、妊婦の未受診抑制、子宮内胎児発育遅延症例の早期紹介、超緊急の分娩体制の確立が課題としてあげられた。新生児死亡回避のための課題として適切な新生児蘇生の施行があげられた。また2021年の滋賀県の周産期死亡率は1.68/1000出生であり全国平均（3.38/1000出生）に比べて最も低かった。滋賀県における各指標の経年的推移を全国平均値と比較したこところ、後期死産率は経年的に大きく低下し、2016年および2021年に全国最低値であった。新生児死亡率は全国平均付近に低下する傾向がみられた。次に、2007年から2021年の15年間を、5年毎の3期（前期、中期、後期）に分けて滋賀県の周産期死亡関連指標を全国平均と比較した。各期間における死産率および新生児死亡率の県別平均値を散布図上で検討した。その結果、滋賀県は前期において死産率および新生児死亡率ともに全国平均を大きく上回っていたが、中期では両指標は全国平均に近づき、後期には新生児死亡率は全国平均付近、死産率は全国平均を下回った。この結果は本研究施行前のデータとして今後の研究結果を評価する上で有用な比較対象となりうるため、来年度以降も周産期死亡症例検討により死亡回避のための提言を地域の医療従事者へ啓発することで、回避が可能な周産期死亡が減少する効果が期待できる。

The purpose of this study was to investigate the effects of death avoidance on maternal mortality in pregnant women. In 2022, there were 4 cases (April 21, May 26, June 23, July 21, 2022), and 4 cases of perinatal deaths (21 late stillbirths and 11 neonatal deaths) in 2020. Analysis of cases can not be divided into two possible cases to avoid judgment. Avoidance of possible stillbirth cases, suggestion of stillbirth avoidance, rapid diagnosis of decreased fetal movement in pregnant women, inhibition of undiagnosed pregnancy, early introduction of delayed intrauterine fetal development cases, establishment of emergency delivery system. The problem of newborn death avoidance is related to the implementation of newborn death. In 2021, the perinatal mortality rate in Shiga Prefecture was 1.68/1000 births, which was the lowest in the country (3.38/1000 births) The year-on-year progress of various indicators in Shiga Prefecture is compared with the national average. The late stillbirth rate is lower than that in the previous year. The lowest value in the country is between 2016 and 2021. Newborn mortality tends to be lower than the national average. A comparison of mortality related indicators in Shiga Prefecture in three periods (early, middle and late) of the 15 years from 2007 to 2021 The mortality rate and neonatal mortality rate of each period are discussed on average. The results show that the mortality rate in Shiga Prefecture in the early stage is close to the national average, and the mortality rate in the middle stage is close to the national average. These results are useful for evaluating the results of previous studies and future studies, and are expected to reduce perinatal mortality in the coming year by reducing perinatal mortality.
