项目摘要

今後急増する心不全において患者が有する症状は多岐にわたるが、その中でも掻痒感は、不眠、QOLの低下を引き起こす負担度が高い症状であり、不眠により心イベント発症リスクは増加する。しかし、現在までに心不全患者に対する掻痒感の緩和のための援助方法、看護技術は検討されていない。心不全の発症・重症化予防のための療養環境の整備が叫ばれている中、心不全の重症化リスク軽減につながる看護技術の向上は重要な課題である。本研究の目的は、心不全患者の掻痒感の緩和を図るための看護技術の確立と効果の検証を行い、さらに普及に向けた教材を作成することである。そのことにより心不全患者に対する看護師の症状緩和の技術を向上させ、今後急増する心不全患者の重症化リスクの軽減、QOL向上に貢献することを目指している。令和3（2021）年度より、掻痒感緩和のための看護技術の検討に向けて、まず対象となる心不全患者の皮膚状態と掻痒感の評価を行ってきた。外来受診をしている心不全患者を調査対象として、皮膚状態（皮膚油分、水分、PH）、皮膚状態に関連したQOL、掻痒感の有無、掻痒の状況、掻痒に関連する因子、疾患に関連する因子等の測定および調査を行った。単年度で調査を終了する予定であったが、COVID-19感染流行の影響により、当初目標とした対象者数への調査が完了しなかったため、令和4（2022）年度も引き続き外来受診をしている心不全患者への調査を行い、目標の対象者数を達成した。中間分析を行ったところ、心不全患者の皮膚状態の詳細な評価のためには、新たに健常者での皮膚状態の調査が必要となったため、当初の計画に加えて健常者での皮膚状態および掻痒感の調査を実施した。現在、調査によって得られた評価項目において分析を進めているところである。
