项目摘要

本研究目的は、がん遺伝子パネル検査を受ける患者の世界（体験）を明らかにすることである。パネル検査から治療実施するまでには、①がん遺伝子パネル検査の説明時、②検査結果の告知時、③治験・臨床検査実施あるいは治療の中止のポイントがある。この一連の過程を参加観察と対象者への面談を通して、心の揺れを含む患者の世界を明らかにする。その上で、パネル検査説明、治療への看護における課題を明らかにする。令和4年度は、コロナの流行の関係で、２カ所の大学病院から紹介のあった対象者は１名のみであった。令和２年度から本年度までに実施した対象者は、計１２名で、その分析結果を、日本がん看護学会で報告するのみに終わった。結果：他の治療がない】【病状改善への期待】【後悔したくない】【周囲の勧め】【今後の医療のため】【検査の負担が少ない】といった動機から受検を決めていた。その一方で、患者は意思決定を行うまでに【検査結果に関すること】【費用負担に関すること】【検体採取に関すること】などといった戸惑いを抱いていた。その戸惑いを解決するために患者は、【治療について情報収集を行う】【治療について医師に相談する】などという行動をとっていた。二次的所見に対しては、【家族性腫瘍の懸念】【知り得ることは全て知りたい】などという理由から開示を希望していた。また、検査結果の説明を受け、言葉を失う者や医師に今後について尋ねる者がいた。考察：看護師は、医師との情報共有を行うと共に、説明の場への同席やその前後で患者のパネル検査の認識や戸惑い等を理解し、それに応じた支援を行うことが必要である。また、特に二次的所見の開示については、患者自身だけではなく、家族の思いを含めた意思決定ができるように支援していくことの重要性が示唆された。
